What to Bring β Enrollment Checklist
To complete an application, please have the following for each person in your household applying for coverage:
β’ Dates of birth
β’ Social Security numbers
β’ Immigration documents (visa, green card, etc.) if applicable
β’ Most recent tax documents (W-2, 1040, 1099)
β’ Paystubs or income details (Profit & Loss statement if self-employed)
β’ Information on current health insurance (if any)
